During startup of the Isomerization unit raffinate splitter tower at BP Texas City, operators pumped flammable liquid hydrocarbons into the 170-foot tower for 3 hours without opening the bottom export valve. The high-level alarm failed to sound, and the sight-glass was fouled with dark residue, showing level in normal range while the tower overfilled to the top. Hydrocarbons flooded the atmospheric blowdown stack, geysered out the top, and ignited on a running diesel pickup truck (15 fatalities, 180 injuries).
